Behind the Scenes: Building the Ultimate Client Intake Form

We spent months refining our intake form template. Here's what we learned about asking the right questions—and which ones to cut.

When we set out to create our client intake form, we thought it'd take a weekend. Instead, it took a month of testing and feedback from dozens of coaches. The biggest lesson? Less is more.

Most intake forms ask too much. Coaches want to know everything, but clients get overwhelmed and give shallow answers. We cut our form from 30 questions to 10, and the quality of responses doubled. Focus on the essentials: goals, challenges, timeline, and communication style.

Another surprise was the power of multiple-choice questions. Clients often don't know how to articulate their struggles, so giving them options like "time management" or "lack of clarity" yields better data than an open text box.

If you're creating your own form, start by listing what you truly need to know for your first session. Then cut it in half. You can always gather more details later. The form is just the first step in building trust.
